it all comes down to the outcome you want from your Social Media Channels and you choosing a platform which you already use a lot and enjoy doing it.
But I think you can focus on creating short form content that can be posted around Instagram, TikTok and Youtube so 3 channels would be covered easily.
Do you mind sharing your username?

It really depends on what you need social media for. I know many plastic surgeons who use Instagram as their portfolio and actively promote it to their audience. However, there are also surgeons who work in government clinics, and social networks are the same resource for them as for ordinary users.

I suspect Instagram will be your main channel. But IMO it’s worth having a profile setup on each major network. Even if the other social profiles just have a couple of posts that get across your main info and inform people that Instagram is where you’re most active.

Make sure you pin a post to your IG profile with your core info. Like where you’re based and some social proof like your google star rating (if that’s relevant). You want to get across what you do, where you’re based and some proof of your reputation. The social proof is especially important when you’re just starting out and have a low follower count.

If you do want to post to all networks then there’s plenty of tools out there to help you with that. Hopper HQ is an option (but I am biased as I work there)
