Facebook’s algorithms cause many false positives, though obviously not enough for them to seriously harm their own business. They will never tell you what triggers it. There will be people here who claim that you must have done something wrong, that’s the beauty of algorithmic discrimination. Facebook has basically automatized slander and there is nothing you can do about it.
The best thing you can do is appeal and hope your ads account is not banned forever. My advice is not to rush it – when they unlock your account and you start a new campaign, be super extra careful about fulfilling the guidelines and give it some time. Upload your photo ID if necessary.
Be careful about using different browsers/different machines and do not copy an old campaign, or else you might trigger the same bug again. If you’re using Firefox, be sure you to disable their tracking protection. If you have it on, image upload may fail and you may get banned.
