I would give up on this campaign since you’ve already got a lot of low-quality conversions.

1) pause the campaign for now

2) set up/import offline conversions

3) make sure tracking is solid

4) let it collect some clean data

5) rebuild a new campaign from scratch optimizing for offline conversions, not form fills
